FCC Transcripts

To request your official transcript, please download and fill out our transcript request form below:

FCC Transcript Request Form

You may obtain an official copy of your FCC transcript by filling out the above form and sending it to the Registrar via one of the three options below:

1.  Mail: send your request form to:

2.  Fax:  send your request to 321-206-2007 Attn:  Registrar's Office

3.  Email: scan your signed request form and email it to registrar@fcc.edu.

An alternative to the form (if not possible to use) is to write a letter containing the following information:

In your request, please include:

• your name (married and maiden, if applicable)
• your current address
• your signature (required)
• your social security number
• the year you left FCC
• the full name and address of the place where you want your transcript sent

Cost and Payment Information: 

• Transcripts are $5.00 each. 
• We accept cash and credit card.  To pay with a credit card, please click here.  **Please note:  the online payment option is not the same thing as a transcript request.  Your transcript will not be sent until the Registrar's office receives a signed transcript request form.
• As of August 15, 2008, we no longer accept personal checks.  We apologize for any inconvenience.

SPECIAL NOTES: 

-Faxed requests are honored, but please understand that we will still have to wait for the fee to be received before we can process the request.

-Because a signature is required for the release of transcripts, E-mail and telephoned requests cannot be honored.

Requesting Transcripts From Other Schools

If you need to request transcripts from other schools you have attended and have them sent to FCC, please feel free to use the form below.  Most high schools and colleges will have their own transcript request forms, but most will accept ours as well.

Please note:  this form is only to be used to request transcripts from other schools; it will not be accepted as a transcript request for your FCC transcripts.
